I, do hereby voluntarily submit my minor child and myself for attendance and participation in Pease Baseball Training / facilities and do hereby assume full responsibility for any and all damages, injuries or losses that she/he may sustain or incur in anyway while attending or participating in any Pease Baseball Training lessons / clinics / facilities. I do hereby waive all claims against the instructors, the owners and/or operators, individually or otherwise, of Pease Baseball Training for any injuries that my minor child or myself may sustain while attending or participating in said programs. For and in consideration of the acceptance of our child/children minor(s) in the programs offered by Pease Baseball we as parents and guardians of such minor(s), so hereby release and forever discharge Pease Baseball, its principals, employees and agents and their heirs, personal representatives, successors and assigns, and save them from harmless from and against any and all claims, actions, liabilities, damages and expenses in connection with loss of life, personal injury and/or damage to or loss of property arising from or out of the participation of such minor(s) in the programs of Pease Baseball. By reading and signing this legally enforceable Release, I/we acknowledge that I/we have carefully read and fully understand the provisions hereof.

Cancellation Policy
We take our schedules very seriously here at the Academy as we are dealing with a significant amount of schedule management and arrangements. Should you ever need to cancel your lesson for whatever reason, you may do so outside of 24 hour time period. If you cancel within 24 hours of your lesson time for ANY reason (other than weather related) the FULL amount of the lesson fee is still due . We will always need 24 hour notice in order to potentially fill your time slot and adjust scheduling. By signing this document you have acknowledged the cancellation policy and will abide by it. If you need to cancel within 24 hours it is expected for you to acknowledge the policy during the time of cancellation with intention to abide by it.
